Referrals/Prior Authorization

THIS NOTICE DESCRIBES YOUR OBLIGATION TO APPLIED DERMATOLOGY, PC CONCERNING REFERRALS. PLEASE REVIEW IT CAREFULLY.

If your insurance plan requires a prior authorization from a primary care doctor, it is your responsibility to obtain and provide us with that authorization prior to your scheduled appointment. To obtain pre-authorization please contact your primary care doctor’s office at least two weeks prior to your appointment. It takes most insurance companies at least one week to receive the referral authorization. Please call us at least three business days prior to your appointment to check that your referral has been received. If prior authorization is not available at the time of your visit you will be responsible for all charges for services rendered, payable in full at the time of service.
