Arriving Late to an Appointment

THIS NOTICE DESCRIBES OUR POLICY FOR ARRIVING LATE TO AN APPOINTMENT. PLEASE REVIEW IT CAREFULLY.

We strive to see each patient at their scheduled time. In order to provide you with the very best health care, we must have your current and past medical health information, and also cannot bill your insurance carrier without a completed demographic profile and a copy of your insurance card. Please arrive early to provide us with or update your information. If you arrive late for your scheduled appointment we reserve the right to reschedule.
